Certificate of vaccination (Vaccine Passport)

The vaccine passport is a document that serves as a record to prove that you have been vaccinated for COVID-19 in Japan. You can apply for one if you…

  1. Are travelling abroad, or
  2. Want to use the certificate in Japan to prove that you have been vaccinated

※1. To see which countries you can use the vaccine passport in, please check the Ministry of Foreign Affair's webpage(外部サイトへリンク)(別ウインドウで開く) , or check with the embassy of the country you want to go to.
※2. For people who want to use the vaccine passport in Japan to prove they have been vaccinated: your vaccine voucher also serves as proof. Please check with the store, location, event, etc. you are attending to see if you can use the vaccine passport or vaccine voucher. You may have to also present a personal ID document (residence/zairyū card, passport, etc.)

How to apply

People with a MyNumber card can use an app on their smartphone to apply for a vaccine passport.
This Vaccine Passport will be a digital certificate, not a paper certificate.

If you want a paper Certificate of Vaccination

Put all the necessary documents mentioned below into an envelope, and send them by mail. You can apply for the certificate if you lived in Chiba City at the time of your vaccination.

※Please be aware that the certificate cannot be issued by city hall or a ward office.

Necessary documents

  • Issue form
    Print out and fill out the application form (PDF(PDF:98KB) or Excel(エクセル:49KB))  or get a copy at a ward office.
  • Personal identification documents
    1. People going abroad
    A copy of your passport
    *A photocopy of the page of your passport that has your photo, your name, and your passport number.
    2. If you want to use your vaccine certificate in Japan!
    A copy of your driver’s license, MyNumber card, passport, health insurance card, etc.
    *Something with your name and birthday printed on it
  •  A copy of a document that proves you have been vaccinated for COVID-19
    This can be the vaccine voucher (接種券, sesshūken) sent to you by Chiba City that you used when getting your vaccine. If you work in a hospital or similar facility, you can use a copy of your vaccination record (接種 記録書, sesshū kiroku sho).
  • A return envelope to send the vaccine passport in
    Write your address on it and stick an appropriately-priced stamp on it (this envelope will be mailed back to you with your vaccine passport). The Vaccine Passport is one page. (A stamp for sending regular mail, around 5g, is 84 yen)
    *You may choose to send it via regular mail or simplified registered mail.

 

Necessary documents (only for people who meet the conditions)

  • People whose have their former name, or a different name/last name written on their passport
    →A copy of a document that has all names (former names and different names) written on it
    *A copy of your MyNumber card, driver’s license, residence record (jūminhyō), etc.
  •  Different names on your vaccine voucher and your personal ID documents
     →A copy of a document that has both the name on your vaccine voucher and the name on your passport written on it
    *A copy of your residence (zairyū) card, MyNumber card, driver’s license, residence record (jūminhyō), etc.
  • The person who will use the Vaccine Passport and the person actually applying for it are different
    A letter of attorney (委任状, ininjō)(ワード:16KB) written by the person who needs the passport, and a copy of a personal ID document of the person applying on your behalf

 

 Getting the vaccine passport issued is free of charge
*You must pay for the stamps that are needed to mail the application and necessary documents, and to get the vaccine passport mailed to you once the application is processed.


When the Vaccine Passport will be sent to you

It can take up to two weeks for your vaccine passport to be issued after application. Please apply as soon as possible.
※If you are planning to travel abroad within two weeks of applying please contact the Chiba City Coronavirus Vaccine Call Center.
