What You Will Need for a Medical Exam

1.  Health insurance certificate
2.  Your passport or some other form of personal identification (if you subscribe to exchange student insurance or traveler's insurance)
3.  Payment for examination fees
4.  A memo that shows your address and telephone number

Chiba Municipal Hospitals

There are two municipal hospitals in Chiba City.

Chiba Aoba Municipal Hospital

Departments	Internal medicine, clinical psychiatry, emergency intensive care, pediatric care, surgery, orthopedic surgery, dermatology, urology, obstetrics and gynecology, ophthalmology, ear/nose/throat department, rehabilitation, dentistry, and anesthesiology.
Reception hours	8時30分AM - 11時30分AM

Access

From bus stop #6 in front of the East Exit at JR Chiba Station:

1. Take the Chiba City Bus bound for Kawado/Miyako-en, get off at Shiritsu Aoba Byoin (Chiba Aoba Municipal Hospital) after approximately 20 minutes, and walk for 1 minute.

From bus stop #7 in front of the East Exit at JR Chiba Station:

1.  Take the Keisei Bus bound for Daigaku-byoin (university hospital)/Minami-Yahagi, get off at Shiritsu Aoba Byoin (Chiba Aoba Municipal Hospital) after approximately 20 minutes, and walk for 1 minute.
2.  Take the Keisei Bus bound for Daigaku Byoin (university hospital), get off at Chuo Hakubutsu-kan (Natural History Museum) after approximately 15 minutes, and walk for 5 minutes (buses bound for Daigaku-Byoin/Minami-Yahagi and Kawado/Miyako-en from Chiba station pass through Chuo Hakubutsu-kan).

From bus stop #2 in front of the East Exit at JR Chiba Station:

1. Take the Chiba Chuo Bus bound for Chuo Hakubutsu-kan (Natural History Museum), get off at Shiritsu Aoba Byoin (Chiba Aoba Municipal Hospital) after approximately 20 minutes, and walk for 1 minute.

From bus stop #2 in front of the East Exit at JR Soga Station:

1. Take the Kominato Tetsudo Bus or Chiba Chuo Bus bound for Daigaku-byoin (university hospital), get off at Chuo Hakubutsu-kan (Natural History Museum) after approximately 15 minutes, and walk for 4 minutes.

From Keisei Chibadera Station:

1. Walk for approximately 20 minutes (buses from JR Soga Station pass through Chibadera Station).

Chiba Kaihin Municipal Hospital

Taking the Chiba Kaihin Kotsu bus from bus stop #4 in front of the South Exit at JR Shinkemigawa Station on the Sobu Line:

1. Take the bus bound for Kaihin Byoin (Chiba Kaihin Municipal Hospital) and get off at Kaihin Byoin after approximately 20 minutes.
2. Take the bus bound for Chiba Nishi High School for approximately 20 minutes, and get off at Isobe 8-chome and walk 3 minutes.
3. Take the bus bound for Isobe High School, get off at Isobe 8-chome after approximately 20 minutes, and walk for 3 minutes.

Taking the Chiba Kaihin Kotsu bus from bus stop #4 in front of the North Exit at Kemigawahama Station:

1. Take the bus bound for Kaihin Byoin (Chiba Kaihin Municipal Hospital) and get off at Kaihin Byoin after approximately 10 minutes.
2. Take the bus bound for Chiba Nishi High School, get off at Isobe 8-chome after approximately 10 minutes, and walk for 3 minutes.
3. Take the bus bound for Isobe High School, get off at Isobe 8-chome after approximately 10 minutes, and walk for 3 minutes.

If You Use a Medical Facility

Always bring your health insurance certificate and payment for treatment. If you do not bring your health insurance certificate, you will have to pay the full amount out of your own pocket.

In Case of Sudden Illness or Injury During the Night

Please contact the nighttime emergency section at Chiba Kaihin Municipal Hospital.

Medical staff of the surgery section (surgery and orthopedics) work on a shift basis.
